Grats on the custom request, they can be a bit obscure at times to say the least.
 
Firstly I contact the customer and I am usually non business friendly, I found being to business chased them away, but being to informal can do the same so just be you. I think its best you accnowledge that they wrote as soon as you can.
 
Then I ask them what sort of design etc they would like, are they wanting it the same as the atached or a different version... usually gets you past the huh what stage
 
I have not had a bad one yet, so far so good and only lost a few along the way, but on the whole good good fun to do.
 
Not everyone will do what I do I guess, but if they ask for a specific change, I try to make at least 4 new cards on similar lines but variations so they have some choice, but some don't like the time it consumes. I love it so I never mind
