3 Card Categories Asssigned to 1 Card

I need assistance with changing a card that has been designated with 3 categories. It's only showing in 1 category. I suspect this happened when categories got shuffled around.

PID# 1379010

In my opinion the two categories should be:
Occasions\Welcome Back\Welcome Back To Work
Business\Employee Relations Cards\Welcome Back to Work\Office\After Baby\Maternity Leave

I'll let GCU figure this one out.
