Is it possible for someone to tell me if my form was received alright or not? I scanned and sent the form in to the e-mail address supplied, but haven't heard back as to whether I had in fact filled in the form correctly (I think I did, but I never was very good at filling them in).
Plus on my account information it still has this...

Which is making me worried that the form hasn't been dealt with, or was somehow wrong??

I filled it out correctly because I got my payment in the mail last week! Oh...great work by the way...amazing!
