I just had my second custom photo card request. Both times, however, the customer did not notice the link that they click to upload an image.  
 
I went and had a look at the form. I am wondering if it would help if an orange #5 could be placed next to the link for the customer to upload an image. I am suggesting a #5 because I think it would certainly catch the eye of the customer and let them know that they are not done yet. Either that or an exclamation point or something.
