Likely a little of both but in general we are pretty literal.
As for grandparents and name variations we do have some broken out, mostly in Birthday, for names like Nana, Pops, Granny, etc. So look for them, if it's not there put in the traditional relation name category and you should be fine. What you will need to keep in mind is that we only allow you to make the same card with up to 3 name variations if they'll be in the same category.
So you could do the following 3 for example all w/in Grandma / Grandmother subcategory:
Grandma
Grandmother
Grammy
